2025-2026 New Student Open Enrollment Application - When the application has been completed, email the application to danielle.satterfield@eaglepride.net or drop off at the office.

Enrolling a New Student to United Local District - Student is a current resident of United Local Schools.
Login to FinalForms - To update and sign all of your students required forms. (Link Coming Soon)
Student's FinalForms MUST be updated and signed electronically every year. Without these forms submitted, your student can not do the following:
Go on field trips
Participate in school sports
Go on band trips
Extracurricular activities
Please be aware that, in addition to completing the OHSAA Sports Physical Forms. You must also ensure that your student's FinalForms are completed before the start of your athlete's sport season. It is mandatory to have both items completed and submitted to be eligible to participate in sports at United Local School District.
