Sports Physical Form Packet Please be aware that, in addition to completing the OHSAA Sports Physical Packet. You must also ensure that your student's Online Yearly Forms are completed before the start of your athlete's sport season. These forms will be available on our District Website starting June 1, 2024. You can locate these forms by selecting Grades 6-12 Yearly Student Forms. It is mandatory to have both items completed and submitted to be eligible to participate in sports at United Local School District.

 COACHES & VOLUNTEERS

Volunteers help our school in a variety of ways, and we truly appreciate the time and energy our community volunteers give to help our students and staff.  If you are interested in volunteering at United Local, please complete the Volunteer Application and turn it in to the building principal.  There are certain situations when volunteers will have to submit an online background check, obtain approval to volunteer and get and ID badge. Please refer to the Volunteering for Our Schools guidelines. Depending on the situation, volunteers many be required to complete the state Pupil Activity Permit application process.

Pupil Activity Supervisor Checklist  The Ohio Department of Education (ODE) requires any paid or volunteer person serving in the supervision capacity of students who are engaged in physical activity to obtain a Pupil Activity Permit, also referred to as a PAV (ORC 3319.303). The details of this requirement are spelled out in Section 1 (p. 4-5). This  checklist may be saved or printed out to help plan and complete the required steps for the PAV application.
